Cook County COVID-19 Mitigation begins January 3 at Sachs Recreation Center
COVID 19 Operational Update
On December 23, 2021, the Cook County Department of Public Health issued new mitigation orders in response to the current surge of Covid-19 cases in suburban Cook County. Beginning Monday January 3, 2022, all members, guests and program participants ages 5 and older will need to provide proof of being fully vaccinated to enter the Sachs Recreation Center. In addition, anyone age 16 and older must show identification (driver’s license, passport, government ID, work or school ID) that corresponds to the vaccination proof.
SRC Members will need to show proof of being fully vaccinated one time and the information will be stored in their household account.
SRC Daily Guests will need to show proof of being fully vaccinated upon visiting.
SRC Program Participants will need to show proof of being fully vaccinated on the first day of class.
Proof of vaccination can be shown in the following ways:
- A CDC COVID-19 Vaccination Record Card
- Immunization record from the state where the vaccination occurred
- An ‘app’ that provides a digital record of vaccinations
- A digital or physical photo of such a card or record that includes the individual’s name, the vaccine administered and vaccination dates
